Founded in 2020, we bring licensed physical and occupational therapists directly to your home, guided by leadership with board-certified expertise in geriatric care. A core focus of our approach is fall prevention and balance rehabilitation. With a philosophy that falling is not a normal part of aging and that balance can meaningfully improve with targeted intervention, we push the limits of each client's stability, building confidence and reducing fall risk through high-intensity, functional training protocols.
Beyond our work with older adults, we also serve busy professionals, active adults, weekend warriors, stay-at-home parents, and individuals seeking a more personalized healthcare experience. By delivering one-on-one care directly in the home, office, gym, or community setting, we eliminate the time, stress, and inconvenience of traveling to a busy outpatient clinic. Our therapists provide focused, uninterrupted sessions tailored to each client's goals, schedule, and lifestyle.
We also offer comprehensive wellness services designed around a proactive model of healthcare. Rather than waiting for an adverse event to occur, we partner with clients to develop customized exercise programs focused on strength, endurance, flexibility, coordination, and balance. Wellness membership packages are available at discounted rates for adults committed to long-term health.

Getting to a PT or OT clinic is hard when mobility is the problem. We come to you. Your therapist sees the actual stairs you climb every day, the chair you get up from each morning, and the yard you want to get back to. Treatment is built around your real life.
Our founders, Dr. Katie Sage and Dr. Derek Smith, are both Doctors of Physical Therapy and board-certified Geriatric Clinical Specialists (GCS).
Our team also includes Chelsea Schuman, PT, DPT, Kate Moran PT, DPT, and additional licensed physical and occupational therapists. Their areas of expertise vary with high intensity training and coaching, dry needling, lymphedema therapy, pelvic floor therapy, and more.

Yes. We are enrolled Medicare Part B providers. Medicare Advantage PPO plans are likely covered. Home health is a specific level of care that most often occurs after a hospitalization. This is often billed under your inpatient (Medicare Part A) benefits. You typically must be homebound. Roaming Rehab is a mobile OUTPATIENT provider that is billed under Medicare Part B. With outpatient therapy, you do not need to be homebound. We bring excellent, clinic level care to you. We like to say home health helps you survive in the home. We help you thrive in your community.
We work with adults dealing with falls or a fear of falling, arthritis, back and neck pain, knee and hip pain, neurological diagnoses including Parkinson's disease, MS, and stroke, cancer-related weakness and fatigue, diabetes, pulmonary disease, vestibular disorders, lymphedema, general deconditioning, and recovery from joint replacements and cardiac events. We also work with busy professionals, active adults, and anyone who wants focused, one-on-one care without the commute to a clinic. GCS stands for Geriatric Clinical Specialist, a board certification awarded by the American Board of Physical Therapy Specialties. It requires passing a rigorous board examination and demonstrates advanced clinical expertise in treating older adults. Most physical therapists do not hold this credential.
