Most healthcare is built around a reactive model: wait until something goes wrong, then seek help. At Roaming Rehab, we believe in flipping that model. Taking a proactive approach to your health will help you achieve the quality of life you desire, save on healthcare costs in the long run, and help prevent bigger issues from arising down the road.

We come to you, 1-on-1, in the comfort of your home. We want to help place you in control of your health so you can live your life the way you want.

What We Do

What Is Physical Wellness?

Wellness is an active, ongoing process of making choices that lead to a healthy and fulfilling life. It spans seven dimensions: physical, emotional, social, intellectual, spiritual, occupational, and environmental. And it extends well beyond the absence of illness or injury.

At Roaming Rehab, our clinical expertise centers on physical wellness: the dimension most directly shaped by movement, strength, balance, flexibility, and cardiovascular health. Physical wellness goes deeper than fitness. It means having the functional capacity to do the things that matter most to you, like climbing stairs, playing with your grandchildren, gardening, walking your dog, or returning to a sport you love.

Physical wellness is also the dimension most predictably improved through structured, evidence-informed intervention. That is where a licensed physical therapist makes a measurable difference, and that is what we do, in your home, on your schedule.

Dealing with an injury, recovering from surgery, or managing a specific diagnosis? Our skilled in-home physical therapy services may be the right fit.

Our Approach

4 Tips for Being Proactive About Your Health

Traditional healthcare often waits for a problem before intervening. By the time a fall occurs, a chronic pain condition develops, or mobility declines, the path back takes far more time, cost, and effort. A proactive wellness model reverses that timeline. By assessing your current strength, balance, mobility, and movement patterns now, and by putting a targeted exercise program in place, we can help identify and address risk factors before they become clinical events.

PCP Check-ins

Check in with your primary care physician on a regular basis, at least yearly. These visits help track lab values, go over medications, complete appropriate health screenings, and discuss any health concerns. Our in-home wellness evaluations complement your medical team's work by focusing on the functional and movement dimensions that clinic visits rarely have time to address.

Structured Exercise

This is where Roaming Rehab comes in! A body in motion stays in motion! Regular exercise is just as important as taking your medications, getting enough sleep, and supplying your body with adequate nutrition and hydration. We can help create and implement an effective exercise program targeting strength, endurance, flexibility, coordination, and balance. This can help prevent pain, falls, cardiac events, and more. We want to help you keep doing the things you love and live your life to the fullest.

Sleep Hygiene

Getting sufficient sleep is important for concentration, productivity, physical performance, immune function, and mental health, and it can even help lower your risk for medical issues like heart disease. As licensed physical therapists, we can help educate you on steps you can take to have appropriate sleep hygiene and support your body's ability to rest and recover.

Nutrition and Hydration

Fueling your body with adequate nutrition and hydration supports physical performance, helps manage your weight, and helps you move better. We can provide general information on a healthy diet and hydration, but do not be afraid to ask your primary care physician or a dietitian about a more structured plan.

Why In-Home

In-Home Wellness: Why Location Matters

Wellness assessments delivered in a clinic tell us what you can do in a controlled environment. In-home wellness services tell us something richer: how you move, function, and navigate your actual life.

An in-home evaluation lets us observe your home environment directly, identifying fall hazards, assessing how your furniture and layout affect your movement patterns, and understanding the specific physical demands of your daily activities. That context makes our goal-setting more precise, our exercise programming more relevant, and our outcomes more meaningful.

For older adults, individuals managing chronic conditions, or anyone returning from surgery or illness, in-home wellness support removes the transportation barrier that often prevents people from accessing consistent care. You do not need to be injured or recovering to benefit. You simply need to care about staying well.

What to Expect

Your Wellness Initial Evaluation

Your wellness program starts with a comprehensive, in-home evaluation. During this visit, your physical therapist will work through three phases with you:

Review

We go over your complete health history and current medications, discuss previous injuries or surgeries, and talk through your personal wellness goals and the activities most important to your quality of life.

Assess

We evaluate your strength, balance, and mobility using functional testing tools to establish a baseline that tracks your progress over time. We also assess your home environment for fall hazards and movement patterns.

Plan

Together, we set functional goals, use the same testing to track your progress, and decide on a visit structure that lets you take control of your quality of life.

The wellness initial evaluation is $165. Current Roaming Rehab insurance patients who move into wellness pay no evaluation fee. See visit package pricing.

Have Questions?

Frequently Asked Questions About Wellness Services

Skilled physical therapy at Roaming Rehab is typically recommended following injury, surgery, or a specific medical diagnosis. Wellness services are designed for individuals who are not currently dealing with an acute medical condition but want to proactively maintain or improve their physical health, strength, balance, and overall function. Both are delivered in-home by licensed physical therapists.

Wellness services generally do not require a physician's referral, though we recommend discussing any new exercise or wellness program with your primary care physician, especially if you manage chronic health conditions. Please contact us to confirm current requirements based on your situation.

No. Wellness services are private pay and are not billed to Medicare or commercial insurance, because they do not require a medical diagnosis. If you are managing an injury, surgery, or diagnosis, our skilled physical therapy may be covered by insurance instead. Call 303-720-4244 to talk through which service fits your situation.

The wellness initial evaluation is $165, and a single wellness visit is $165. Visit packages lower the cost per visit: 4 visits are $145 each ($580 total) and 8 visits are $120 each ($960 total). Wellness is private pay.

Yes. Current Roaming Rehab insurance patients can transition into wellness with no evaluation fee. Talk with your therapist about when the move makes sense and which visit package fits your goals.

The initial evaluation is a comprehensive in-home visit that covers your health history, personal goals, functional assessments of strength, balance, and mobility, and collaborative planning for your wellness program. It establishes your baseline and gives us a clear picture of where to focus.

Visit frequency varies by individual and goal. Some clients benefit from weekly visits during an initial program phase, while others transition to monthly check-ins once a strong exercise routine is established. Your physical therapist will recommend a visit structure during the initial evaluation.

The most widely cited model includes seven dimensions: physical, emotional, social, intellectual, spiritual, occupational, and environmental wellness. Some models also include financial wellness as an eighth dimension. Roaming Rehab's clinical services address physical and environmental wellness directly, while supporting overall quality of life across all dimensions.

The 5-3-1 rule is a practical wellness framework: aim for 5 servings of fruits and vegetables, 3 servings of whole grains, and 1 hour of physical activity per day. While this is a general population guideline, your Roaming Rehab physical therapist can help you build toward the physical activity component and surpass it through a structured, individualized exercise program calibrated to your current capacity.

Yes. Fall prevention is one of the most evidence-supported applications of physical therapy in a wellness context. Balance training, lower extremity strengthening, gait assessment, and home environment modification are all within our clinical scope and directly reduce fall risk, particularly for older adults or those with balance or mobility concerns.

A gym or fitness center provides equipment and space. In-home wellness services provide a licensed physical therapist who designs your program based on your specific health history, goals, and functional limitations and delivers it where you live, with full attention to your home environment. For many people, particularly those managing health conditions, recent surgeries, or mobility limitations, in-home services provide a level of clinical personalization that a gym setting cannot replicate.

Home health therapy is a distinct level of care typically provided following a hospitalization or acute medical event. It is billed under Medicare Part A as an inpatient benefit, and patients are generally required to meet homebound status to qualify. Roaming Rehab is a mobile outpatient provider billed under Medicare Part B. As an outpatient service, you do not need to meet homebound criteria. We bring clinic-quality physical and occupational therapy directly to you. Home health helps you survive in your home. We help you thrive in your community.
