Katie Sage PT

Katie Sage PT, DPT, GCS
Founder, Physical Therapist

I was born and raised in North Dakota. I started my career in healthcare at the age of 15, first becoming a certified nursing and medical assistant. I have always had a passion for working with older adults, and this passion has grown since becoming a physical therapist, realizing how much I can help older adults achieve. 

I graduated from North Dakota State University in 2011 with my Bachelor's of Science degree in Psychology, then went on to earn my Doctor of Physical Therapy degree from Regis University in Denver in 2014.

My physical therapy practice has been focused in Geriatrics, primarily in the home setting. Since 2016 I have participated in a research study through the NIH focusing on higher intensity exercise to improve function in older adults. This experience helped me realize how important it is to push the envelope and really challenge people in order to see good results.

​In my free time I like to take my dog Banjo hiking, do Crossfit, and go to concerts at Red Rocks.

I am very excited to have the opportunity to work with you and change the way people look at the aging process! ​
Derek Smith PT

Derek Smith PT, DPT, GCS
Founder, Physical Therapist

I was born and raised in Buffalo, NY. I attended the University at Buffalo for undergraduate and graduate school where I graduated with my Doctor of Physical Therapy degree in 2012. Through 15 years of playing rugby, I have learned the importance of rehab and keeping yourself fit and healthy. 

My career has mainly focused in Geriatrics and older adult care in the Home Health setting. Over the last few years, I have been lucky to take part in an NIH study focusing on higher intensity exercise to improve function in older adults. I have taken multiple continuing education courses aimed at higher intensity therapy (read: fun!) for older adults which has changed my way of practicing immensely!

I moved to Colorado in 2013 and live with my incredible wife, Isable, and baby girl, Imogene, in Lakewood (with our dogs, Lucy and Kolby, and cats, Jonesy and Marley).  We enjoy hiking and any outdoor adventure we can find. In 2022, we finished climbing all 58 of Colorado's 14ers! 

I love practicing in a person's home. This incredible setting has helped me build relationships with people that are so unique and difficult to capture elsewhere.

Chelsea PT

Chelsea Schuman PT, DPT
Physical Therapist

I am a transplant to Colorado from North Carolina, however I was born and raised in Upstate New York. I earned my doctorate degree in Physical Therapy from SUNY Upstate Medical University in 2018.

I have spent the first five years of my career in outpatient orthopedic clinics working with a variety of diagnoses and patient populations. Through those five years I have honed my skills in post op and general orthopedic issue recovery and chronic pain education. Over the last year I have grown a passion in helping older adults reach their full potential. Whether that be coming back from injuries, surgeries, or just helping them to increase their strength and function.

My main goal is helping people realize their full potential and achieve things they never thought possible. I believe that to accomplish this there needs to be a good balance of pushing people and meeting that person where they are at in their function.

When not working you can find me anywhere in mountains. I love hiking with my Mini Aussie, snowboarding, and trying out new breweries and restaurants. I also compete in bodybuilding shows and love to challenge my strength in the gym.


I truly look forward to working with you and your patients to help them reach their full potential and recover from injury.

Maddie Margolis PT, DPT

I was born and raised in Minneapolis, MN, and earned my Bachelor of Science in Kinesiology from the University of Minnesota before completing my Doctorate in Physical Therapy at A.T. Still University in Mesa, AZ. After graduating, my husband and I moved to Colorado in 2024 and have loved making it our home.

I began my career in the outpatient orthopedic setting, where I gained experience treating a wide range of diagnoses and learned the value of providing truly personalized care. Over time, I developed a strong passion for working with the aging population. I believe that staying active and maintaining independence is essential for quality of life, and I strive to empower my patients to move in ways that are safe, functional, and meaningful to them. One of my favorite parts of being a physical therapist is helping people accomplish goals they didn’t think were possible.

In addition to my clinical experience, I bring a background as a yoga teacher and personal trainer, and I hold advanced certification in dry needling. I enjoy blending manual therapy, creative exercise, and education to create treatment plans that help each patient reach their full potential.

Outside of work, I love spending time outdoors with my husband and our dog. You can often find us hiking, camping, paddle boarding, or gardening during the warmer months.

Katelyn PT

Katelyn Palazzolo PT, DPT, OCS, FAAOMPT
Physical Therapist

I was born in St. Louis, MO, and developed an interest in rehabilitation early on, having gone to PT for various injuries when I played competitive volleyball. I got my bachelors in Exercise Science at Truman State University in 2012 before going to Creighton University and earning my Doctorate of Physical Therapy in 2015. 

I am a board-certified orthopedic specialist and have a fellowship in manual therapy. While I love working in orthopedics, I also have a  passion for educating patients about their bodies and working together to solve problems. Aging is a normal part of life, but all too often the fear of falling or losing physical function keeps individuals from thriving. I love helping people find the right combination of knowledge, physical fitness, and mental resilience to stay "young." 

I spent the last three years as a travel therapist working across multiple states, but I'm happy to be back where I started my career in Denver! When I'm not treating patients, you can find me running, traveling, decorating cookies, or indulging in Colorado's craft beer scene. 
Beth R PT

Beth Ruminski PT, DPT
Physical Therapist

I was raised in Colorado Springs, and I attended Gonzaga University in Spokane, Washington for my undergraduate degree in Exercise Science. 

After earning my Doctorate in Physical Therapy from Creighton University in Omaha, Nebraska in 2015, I devoted my early career to the treatment of women’s and men’s pelvic health, as well as management of orthopedic surgeries, pain, and balance disorders.

For the past 6 years, I have been back in Denver and treating patients in the Home Health setting. Throughout this time, I gained additional training in Parkinson’s Disease through the LSVT BIG certification, as well as cardiovascular and pulmonary issues that affect the older adult. I love the unique experience of meeting clients at their homes in order to truly learn what motivates them.

I love to go to Rockies games, play volleyball in the park, and spend time with my husband, dog, and friends outside and always with good food! I am passionate about bringing creativity, motivation, and knowledge of what the body is capable of, in order to help you find your strength and reach your goals!

Beth K PT

Beth Konopka PT, DPT
Physical Therapist

I am dedicated physical therapist who graduated with a Doctor of Physical Therapy degree from the University of Florida in 2014.

I have a background working in both outpatient PT clinics and home health, where I gained extensive experience treating patients using a combination of modalities, therapeutic exercise, and manual therapy techniques.

I have a strong commitment to helping patients reach their goals and improve their quality of life.

​In my free time, I love spending time outdoors with friends and family doing activities such as running, hiking, biking, and snowboarding.

Michelle Kinnan OTR/L, CLT-LANA
Occupational Therapist

​I was born in Texas and raised in a small town in Missouri. I received my masters in Occupational Therapy at the University of Missouri – Columbia in 2009. I started my career in a rural hospital prior to moving to Denver, CO in 2011. I have since lived in Portland, OR and happy to call Denver my home again. 
 
As a therapist I have worked in orthopedics, cardiac intensive care, trauma, burns and mental health. I am a board certified lymphedema therapist, treating patients since 2017. I have experience working with all stages of lymphedema and edemas related to post surgical, burns, trauma, cancer and chronic venous insufficiency. It is important to me to work with you and your health care team to meet your personalized goals for improved quality of life, independence and overall well-being.
 
During my free time I love spending time with my husband, two kids and chocolate lab. As a family we enjoy spending our time traveling, cooking, gardening, fishing and camping.

Catherine Daas PT, DPT
Physical Therapist

I have been practicing as a physical therapist since 2019, working in clinics, hospitals, and homes. I am passionate about empowering people to reach their therapy goals, and I have extensive experience helping patients recover from surgeries, falls, and other conditions that limit movement.

​Outside of work, I enjoy spending time with my husband and our two dogs. I am an avid outdoors person. I like to run, climb, and ski.

Lindsey Boshears PT, DPT
Physical Therapist

I am originally from Fargo, North Dakota (yes like the movie!) and I went to undergrad at the University of Minnesota. After a semester abroad in Spain, I came back and went to grad school for Physical Therapy at the University of Southern California.

After working a few years in an outpatient orthopedic setting in California, I moved to Colorado in 2018 and I switched to home health and have never looked back. I enjoy working with older adults in their home and find that it is much more personable and effective. I am also certified in dry needling and have found fantastic results with utilizing this treatment for my patients.

I met my now husband here in Denver in 2019 and we now have two kids, a dog, two cats, and nine chickens! Life is a bit crazy at times but I wouldn’t have it any other way. I love staying active by doing Crossfit and snowboarding and as a family we love to go camping and fishing together!

Ava Steffen PT, DPT
Physical Therapist

I was born and raised in Aurora, Colorado. I am a University of Colorado grad (GO BUFFS!), and received my Doctorate of Physical Therapy at the University of St. Augustine in San Marcos, CA

I have always known I wanted to be in the medical field, and PT was always the most interesting to me. I was able to gain a lot of experience prior to PT school as an aide in inpatient rehabs, outpatient orthopedic clinics and at Craig Hospital for brain and spinal cord injuries in Englewood, CO.

Since I graduated from PT school in 2020, I have had the opportunity to travel all around California and Washington as a travel physical therapist. This allowed me to see many different settings and work with many different diagnoses and patient populations. I have always had a passion for working with older adults and assisting with reaching their full potential and helping you to realize what your body is capable of!

I moved back to Colorado to be closer to my family after having a baby boy in May of 2024. In my free time, I love to do olympic weightlifting, go to sporting events, snowboard, hike, read, and hang out with my husband and baby!

Bridget Woolley OTR/L, CTM/CM
Occupational Therapist

I was born and raised in Colorado, primarily on the Western Slope, and I earned a degree in Occupational Therapy from Colorado State University.

I have worked in both civilian and military settings, helping individuals recover from a wide range of challenges. My expertise lies in complex rehabilitation areas, including neurorehabilitation, prosthetic training, burn recovery, cardiac rehab, and traumatic brain injury. I utilize a combination of evidence-based therapy techniques and occupational therapy-focused care such as adaptive strategies, cognitive retraining, and activities of daily living (ADL) rehabilitation. 

I truly enjoy working with individuals of all ages, but I have a special passion for helping older adults maintain their independence and confidence. Aging is a natural part of life, but concerns about balance, mobility, and independence often hold people back from fully enjoying it. My goal is to help individuals build confidence in their abilities by combining education, movement, and practical strategies that support long-term wellness. I take a hands-on, personalized approach to care and have found that working with patients in their homes leads to more effective and meaningful results.

My husband and I have an amazing son who has encompassed our life with his love for hockey. As a family, we stay active in the community, often accompanied by our working therapy Labradors, who play a special role in the healing process.

Angie Nguyen, PT, DPT

I have always been passionate about being healthy and helping others. Practicing physical therapy allows me to live my passion to the fullest.  I have worked in outpatient orthopedics since graduating PT school in 2000.  Since then,  I have developed an integrative hands-on approach that gives each patient the individualized attention they need, so they get can get better care and faster results.  I have earned certifications in Manual Therapy, Dry Needling (Level 1 & 2), Pilates for Rehabilitation, Yoga, and Kinesiotaping. 

I can treat most orthopedic conditions of the spine or extremities.  My treatments include: manual therapy,  dry needling,  IASTM (instrument assisted soft tissue mobilization),  pre/post-operative rehabilitation, therapeutic modalities, exercise prescription, mat/reformer pilates, yoga, cupping therapy, kinesiotaping, postural assessments, body mechanics education, gait analysis, and custom orthotics.

Amber Duncan OTR/L
Occupational Therapist

I was born and raised in Philadelphia, PA where I obtained my Masters in Occupational Therapy at Salus University. I moved to Colorado in 2022 to be closer to the mountains and have more access to outdoor recreation.

I have been practicing as an occupational therapist for over seven years with experience in settings such as independent living, assisted living, skilled nursing, and home health. I became LSVT BIG® certified in 2020 in order to be able to better assist patients living with Parkinson's Disease and other neurological disorders. I am passionate about supporting the community and helping my patients stay active, independent, and engaged in the things that bring them joy.

Outside of work, you’ll often find me exploring the outdoors—whether hiking with my husband and dog, camping under the stars, or getting creative with my latest crafting project. I try to bring that same enthusiasm and care into my therapy sessions, creating a welcoming and encouraging environment for every patient I work with.

Kevin Wood PT, DPT

Originally from Kentucky, and adventuring in Colorado since 2016, my rehab career grew out of 10 years of strength coaching in old weightrooms, private studios, and living rooms. Wherever the location, my passion is using movement training to empower adults to do what they love, free from pain and setbacks.
 

Always seeking the best education, science, and systems to give my clients every advantage, I specialized in Senior Fitness Training and Corrective Exercise. These disciplines inspired me to become a PT and earn my Doctorate of Physical Therapy from Hanover College in 2024. The magic happens when we take the knowledge of how to get you back in action with the ability to make the process approachable so we can take meaningful steps forward each day. 

A day outside is a day well spent and my wife and I take as many as we can for new adventures! We can be found enjoying live music or hiking and camping with our two dogs. Between snowboarding, paddleboarding and longboarding, there’s a ride for every season and I’m always out chasing new terrain.

Kerri Tiller PT, DPT, OCS, CSCS

I am originally from Pennsylvania and relocated to Colorado after graduating PT school to be closer to the mountains.

I have been practicing physical therapy since 2013 and have worked with a variety of conditions in general outpatient settings.  I continued my training after PT school to gain a specialization in orthopedics (OCS) and strength and conditioning (CSCS). I enjoy taking a multifaceted approach to helping patients reach their goals while resolving pain and limitations. I utilize manual therapy techniques such as joint mobilization/manipulation, dry needling, cupping, IASTM, and exercise aimed at improving strength, balance and coordination.
​

In my free time, I enjoy spending time with my family and am also an avid rock climber. I spend time working on outdoor climbing routes and coaching climbing at local gyms in the area.
